### How It Works

Simple process:
1. State the problem
2. Ask "Why did this happen?"
3. Ask "Why?" again (to the answer)
4. Repeat until you reach root cause
5. Usually takes 5 iterations (hence "5 Whys")

SOLUTION: Add a filter (prevents future occurrences).
